This standard form specified in Appendix 6 (Form.1), Circular No. 10/2012/TT-BXD dated 20 December 2012 of the Ministry of Construction.
2.
Written approval of newly-established company on foundation construction method (original)
This is prepared by the investor and supplements the application file in case new building(s) will be put up whose basement are adjacent to the existing building(s). This aims to ensure the safety of the additional constructions.

Authenticated copy of investment certificate***

Copy of certificate on fire prevention and fighting
It is applicable to the construction specified in Appendix 3a, Decree No. 46/2012/ND-CP dated 22 May 2012 of the Government.

It is applicable to the construction specified in Appendix 3a, Decree No. 46/2012/ND-CP dated 22 May 2012 of the Government.

This is optional and provided by the construction design company. This must clearly and convincingly demonstrate the capability and experience of consulting unit in construction design.
In case of the construction located in the Area I of Tam Dao Town, Tay Thien Festival Cultural Center or Dai Lai Resort Area, please supplement:
1.
Agreement on total area of construction and architectural plan(s) (an authentic copy)
This is applicable to the construction in Area I, Tam Dao Town; the construction in Tay Thien Festival Cultural Center and Dai Lai Resort within the scope of construction planning with a scale of 1/2000 – in accordance with Clause 4, Article 9, Decision No. 10/2013/QD-UBND dated 28 May 2013. If the project is subject to this case, investor should further contact the Department of Construction (add: #2, Ly Thai To Str., Dong Da Ward, Vinh Phuc Province, phone: +84 211 3862 584, email: soxd@vinhphuc.gov.vn) to obtain this document before applying for construction permit at IPA.
Time frame
Pursuant to Clause 6, Article 9, Decree No. 64/2012/ND-CP, the Government, the time-limit for result issuance should be no later than 20 working days from the date of receipt of a valid application file.
In Vinh Phuc, for the purpose of investment encouragement and administrative procedure reform, this time-limit has been reduced to 10 working days (equivalent to 50% of the time-limit required by the Government). In fact, waiting time for result issuance may extend to 10 working days in case of incomplete application file which needs amendment and supplementation at the request of the Department.

Additional information
IPA shall receive application file and check their validity in compliance with the laws; Later, IPA will give the document receipt and transfer slip to the applicant if the application file is valid. Otherwise, IPA will guide the applicant to re-prepare and complete the application file.
Within 10 working days from the date of receipt of valid application file, the application file shall be transferred to the Department of Construction for reviewing of its contents and doing field inspection (if necessary).
While reviewing, the Department of Construction may require, in writing, the company/investor to amend and/or supplement necessary document(s).
In case the amendment/supplementary document(s) do not meet the requirements indicated in the 1st written request, the 2nd request shall be issued by the Department of Construction to provide further guidelines for company/investor to complete the application file.
Note that if the company/investor keeps on failing to complete the application file as required upon the second request, the Department of Construction shall reserve the right to refuse the issuance of the license and issue a refusal notice to company/investor.
